		abstract={The Department of Religion does not have an employee appraisal information system, so that all employee assessment activities have not been well accommodated, such as employee data, job data, work unit data, performance appraisal data, and other data. System design is done by modeling language DFD (Data Flow Diagram) with system testing apply black-box testing with functional testing and error handling testing.
