While in indonesia, it is reported that there 7.389 case of maternal deaths in 2021. Additinally, in palembang, there were 20 people also experienced the maternal mortality case in 2019. By the occured cases, midwives seem to play important roles in reducing the Maternal Mortality Rate (MMR) and infant Mortality Rate (IMR) which would focus on early detection of complications and normal birth assistence. The purpose of providing continuity of care (COC) is started from pregnancy, childbirth, postpartum, and neonates as an effort to reduce MMR, in this study, midwifery continuity of care for Mrs."E" in the midwife "E" independent practice was done from October 3,2022 to May 11, 2023. This study belongs to a case study by implementing primary and secondary data collection technique. The result were portrayed descriptively by using SOAP (Subjective, Objective, Assesment, Plan) note method. It was found out that Mrs."E" experienced a full-term pregnancy, normal delivery, normal newborwn, postpartum, and neonatal in a physiological sense. In conclusion, the independent practice is recommended to maintain and improve the quality of the midwifery continuty of care. Keywords: Continuity Of Care (COC).} }
