It is higher than the number in high-income countries which reach 11/100,000 LB. The Palembang City Health Department stated that the MMR was 4 cases/26,837 LB and the Infant Mortality Rate (IMR) was 24 cases in 2018. Midwifery care of Continuity of Care (COC) is a sustainable midwifery care for a mother during pregnancy, labor, and postpartum periods which aims to prevent complication risks during periods. The COC is also expected to decrease the number of MMR and IMR. This final report provided midwifery care of COC to Mrs. ?L? at Midwife Ellna?s Independent Midwifery Practice Palembang from October 6, 2021 to March 18, 2022. The COC was conducted from the pregnancy, labor, newborn baby, postpartum to neonate. Primary and secondary data were used to collect the data. Subjective, Objective, Assessment, and Plan note was employed in this case study. The result revealed that the pregnancy, labor, and newborn baby were normal. Meanwhile, the postpartum and neonate were in physiological scope. The independent midwifery practice is recommended to maintain and improve the quality of midwifery service.
