In Indonesia, it was estimated that the number of MMR was 4,627/100,000 Live Births (LB), and the number of Infant Mortality Rate (IMR) was 20,266/LB in 2020. Providing Continuity of Care (COC) is one of the efforts to reduce the number of MMR and IMR. The aim of this final report is to carry out midwifery care of COC to Mrs. ?R? from pregnancy, labor, newborn baby, postpartum, neonate and family planning at Midwife Ellna?s Independent Midwifery Practice Palembang. This final report used study case design of midwifery care of COC and was documented in Subjective, Objective, Assessment, and Plan (SOAP) note. It was conducted at Midwife Ellna?s Independent Midwifery Practice Palembang from January 28, 2022 to May 3, 2022 and started from pregnancy, labor, postpartum, and neonate. Primary and secondary data were used to collect the data. The results indicated that from the pregnancy history, Mrs. ?R? suffered cough in the first and second trimesters, no complaint in the third trimester, and 6- day postpartum and 6-day neonate were categorized in normal level. However, in the first labor stage, a referral assessment was conducted with assessment G2P1A0, 38? 39-week pregnancy with KPSW. Therefore, there was no labor care in the second, third, and fourth of labor stages, 6-hour postpartum, and 6-hour neonate. The conclusion of midwifery care of COC to Mrs. ?R? is the plan, implementation, and evaluation are carried out based on subjective and objective data. The patient is expected to consistently maintain health of their body and baby.} }
