Meanwhile, in 2012 it reached 358/100,000 LB. The number of Infant Mortality Rate (IMR) in Indonesia also decreased from 2012 to 2017. In 2012, the number of IMR was 32/1,000 LB, while, it was 24/1,000 LB in 2017. Continuity of Care (COC) is a sustainable service to a mother which aims to reduce the complication risk during the pregnancy, labor and postpartum. This final report provided midwifery care of COC to Mrs. ?S? at Ellna?s Independent Midwifery Service, Palembang from November 23, 2021 to April 13, 2022. The COC was conducted from pregnancy, labor, newborn baby, postpartum, neonate, to family planning. Primary and secondary data were used to collect the data. This study case used Subject, Object, Assessment, and Plan (SOAP) note. The result indicated that the condition of Mrs. ?S? from the pregnancy, labor, newborn baby, postpartum, neonate, and family planning was in physiological scope. In conclusion, the subjective and objective data inferred that the pregnancy, labor, newborn baby, postpartum, neonate, and family planning were normal. Plan is set based on the assessment. It is recommended that the patient consistently maintain health and hygiene both for themselves and the baby.} }
