Doctors in hospital are the backbone of the hospital because they are the main actor in providing healthcare for patients to cure them from their diseases. Communication skill plays an important role for doctor-patient communication since it can lead to a successful action in helping patient solve their problems or diseases; therefore, this skill is a must for every doctor to possess. However, the input from patients poses evidence that communication skill has successfully been applied by a doctor well in a communication process. In fact, there are doctors that solely present self-oriented communication; thus, they lack sensitiveness in understanding patients’ fear or doubt in doctor-patient communication. In addition, doctors sometimes explain a case to their patients with their own communication style which in fact cannot be understood fully by the patients. Therefore, a doctor should comprehend components in interpersonal communication, comprising source, message, media, recipient, and feedback so that doctor-patient communication regarding health issues can effectively be done. Objective: To find out how doctor-patient communication regarding health issues is applied and whether effective communication is realized. Method: This was a descriptive study with qualitative approach in nature that resulted in descriptive data in form of words both written and orally from the respondents and their observed behavior. The instrument of this study was observation sheet and in depth interview to support the primary data. The population was medical doctors and medical specialists. Result: The doctors in Wirosaban local hospital had left old paradigm, which is, conducting communication only asking about disease, giving information concerning patients’ disease only as needed, and ending a consultation without giving complete explanation. They had also involved patients in the next medical action plan or in decision making. However, doctor-patient effective communication regarding health issues is currently not a priority seen from communication process in the hospital.
