Pada dasarnya sistem yaitu suatu kerangka dari prosedur-prosedur yang saling berhubungan, yang disusun sesuai dengan skema yang menyeluruh untuk melaksanakan suatu kegiatan atau fungsi utama dari suatu perusahaan yang dihasilkan oleh suatu proses tertentu yang bertujuan untuk menyediakan suatu informasi. Sedangkan prosedur yaitu  urutan-urutan setiap tugas dilakukan, berhubungan dengan apa, bilamana tugs tersebut dilakukan dan oleh siapa saja tugas harus dilakukan.  Jenis penelitian yang digunakan peneliti adalah penelitian deskrptif dengan pendekatan kualitatif. Data ini diambil dari Unit Rekam Medis yang berada di Rumah Sakit Arsy Paciran dibagian assembling dan Unit Rawat Inap.   Dari hasil observasi menunjukkan bahwa di Rumah Sakit Arsy tentang Standar Prosedur Operasional (SPO) tentang kelengkapan pengisian formulir ringkasan masuk dan keluar pasien rawat inap sudah baik, terbukti sudah adanya SPO mengenai berkas rekam medis dan sudah berjalan kelengkapan dengan baik. Akan tetapi masih ada yang belum terisi kelengkapannya yaitu pada item ?diagnosa akhir dan tandatangan DPJP?. Kesibukan dijadikan alasan utama dokter untuk tidak melengkapi berkas rekam medis rawat inap. Tanggungjawab pengisian kelengkapan berkas rekam medis berada di tangan dokter walaupun perawat hanya menuliskan identitas pasien, namun perawat harus mengisi identitas pasien dengan lengkap.

ABSTRAK 
 
Basically the system is a framework of interrelated procedures, arranged according to a comprehensive scheme to carry out an activity or main function of a company that is produced by a particular process that aims to provide information. While the procedure is the order in which each task is carried out, related to what, when the tugs are performed and by whom the tasks must be performed. This type of research used by researchers is descriptive research with a qualitative approach. This data was taken from the Medical Record Unit in Arsy Paciran Hospital in the assembling section and the Inpatient Unit. From the observations, it shows that at Arsy Hospital on Standard Operating Procedures (SPO) regarding completeness of filling in and out summary forms of inpatients is good, it is proven that there is an SPO regarding medical record files and has been completed properly. However, there are still some things that have not been completed, which is the item "final diagnosis and DPJP signature". Busyness is used as the main reason for doctors not to complete the inpatient medical record file. The responsibility for filling out the completeness of the medical record file lies with the doctor even though the nurse only writes down the patient's identity, but the nurse must fill in the patient's identity completely.
