		abstract={ABSTRACT 
The Cause for Death sheet is a note that explains that someone has died. This record contains the patient's identity, Medical Record Number, length of time the patient was treated until death and the Causes of Death. The authority to publish this cause of death sheet is a doctor who has taken his oath and fulfilled administrative requirements to carry out medical practice. know the importance of completeness of this record in various circles such as heirs (insurance), statistics / census of the hospital as consideration for improving the quality of the hospital. The purpose of this study was to determine the percentage of completeness and incompleteness of the cause of death sheet for each item in the medical records of inpatients at the Kediri Baptist Hospital This type of research is descriptive, data is taken from the medical record unit of the Baptist Hospital in Kediri in the Medical Records Document Storage (Filling) section. The data collected comes from the medical record documents of patients who died in the Early Quarter of 2019, then the percentage of completeness and incompleteness of the cause sheet was calculated. Dead. Data is presented in tabular and graphical form to assess the level of completeness of the cause of death sheet at Kediri Baptist Hospital in the Early Quarter of 2019. From the results of the study, the Percentage of Completeness of the Death Cause Sheet for each item in Kediri Baptist Hospital in the Early Quarter of 2019, starting from the patient's name to the doctor's signature and the doctor's name tends to be incomplete. This can be seen in the percentage of completeness of each item. Patient Name 84.76%, Medical Record Number 83.81%, Age 5.71%, Cause of Death 94.29%, Duration from Pain to Death 25.71%, Signature and Name of Doctor 77.14%. It clearly shows that the level of completeness has not yet reached the 100% complete target.

ABSTRAK 
Lembar Sebab Kematian merupakan catatan yang menerangkan bahwa seseorang telah meninggal dunia. Catatan ini berisikan identitas pasein,Nomor Rekam Medis,Lamanya pasien dirawat hingga meninggal dunia dan Penyebab Kematian. Kewenangan penerbitan lembar sebab kematian ini adalah Dokter yang telah mengambil sumpahnya dan memenuhi syarat administratif untuk menjalankan praktik kedokteran. mengetahui pentingnya kelengkapan catatan ini di berbagai kalangan seperti ahli waris (asuransi),statistik/sensus rumah sakit sebagai bahan pertimbangan untuk meningkatkan kualitas rumah sakit. Tujuan penelitian ini adalah untuk mengetahui prosentase kelengkapan dan ketidak lengkapan  lembar sebab kematian untuk setiap itemnya pada rekam medis pasien rawat inap di rumah sakit baptis Kediri Jenis penelitian ini bersifat deskriptif ,Data diambil dari unit rekam medis 
Rumah Sakit Baptis Kediri dibagian Penyimpanan Dokumen Rekam Medis (Filling).data yang dikumpulkan berasal dari dokumen rekam medis pasien meninggal pada Triwulan Awal Tahun 2019 ,kemudian dilakukan perhitungan prosentase kelengkapan dan ketidaklengkapan lembar sebab kematian. data disajikan dalam bentuk tabel dan grafik untuk menilai tingkat kelengkapan lembar sebab kematian di Rumah Sakit Baptis Kediri pada Triwulan Awal Tahun 2019. Dari hasil penelitian, Prosentase Kelengkapan Lembar Sebab Kematian untuk setiap itemnya di Rumah Sakit Baptis Kediri pada Triwulan Awaln Tahun 2019, mulai dari Nama Pasien s/d tanda tangan dokter dan nama dokter cenderung belum lengkap. Hal ini dapat dilihat pada Prosentase kelengkapan setiap itemnya. Nama Pasien 84,76%, Nomor Rekam Medis 83,81%, Umur 5,71%, Penyebab Kematian 94,29%, Lamanya Mulai Sakit Hingga Meninggal Dunia 25,71%, Tanda Tangan dan Nama Dokter 77,14%. Jelas menunjukkan kalau tingkat prosentase kelengkapannya belum mencapai target 100% lengkap.
