Completeness and accuracy of filling medical record files by health workers will facilitate other health workers in providing actions and therapy to patients. In addition, it is also a source of information in data processing which will then be useful information for management in determining strategic steps for the development of health services. In this study, the authors used a quantitative method with a descriptive research approach used to examine populations or certain samples, data collection using research instruments, quantitative data analysis or descriptive statistics to assess the completeness and accuracy of filling in-patient medical records. The results of quantitative descriptive analysis showed the completeness and accuracy of filling inpatient medical record files from 100 samples of medical record files obtained the completeness and accuracy of filling medical record files is 58%, while the incomplete and inaccurate medical record file is 42%. Where the results of the completeness and accuracy of filling the medical record file at the Muhammadiyah University Hospital Malang is still low because the standard set by the Minister of Health is 100%. As a conclusion from the results of the above analysis that the completeness and accuracy of filling medical record files at the Muhammadiyah University Hospital Malang is still lacking, it is necessary to make improvements in order to achieve the quality of medical record services. To facilitate officers in assembling and searching for forms in an effort to minimize the level of incompleteness and inaccuracy in filling out medical record files. in need of the provision of restrictions and outreach to medical and non-medical staff in order to foster discipline and Responsibility of officers.

ABSTRAK 
   
Kelengkapan dan keakuratan pengisian berkas rekam medis oleh tenaga kesehatan akan memudahkan tenaga kesehatan lain dalam memberikan tindakan dan terapi kepada pasien. Selain itu juga sebagai sumber informasi dalam pengolahan data yang kemudian akan menjadi informasi berguna bagi pihak manajemen dalam menentukan langkah-langkah strategis untuk pengembangan pelayanan kesehatan. Dalam penelitian ini, penulis menggunakan metode kuantitatif dengan pendekatan penelitian deskriptif yang digunakan untuk meneliti populasi atau sampel tertentu, pengumpulan data menggunakan instrument penelitian, analisis data bersifat kuantitatif atau statistik deskriptif untuk menilai kelengkapan dan keakuratan pengisian rekam medis rawat inap. Hasil analisis deskriptif kuantitatif 
menunjukkan kelengkapan dan keakuratan pengisian berkas rekam medis rawat inap dari 100 sampel berkas rekam medis di dapatkan hasil kelengkapan dan kekuratan pengisian berkas rekam medis adalah 58%, sedangkan berkas rekam medis yang tidak lengkap dan tidak akurat sebesar 42%. Dimana hasil kelengkapan dan keakuratan pengisian berkas rekam medis di RSU Universitas Muhammadiyah Malang masih rendah karena standart yang di tetapkan permenkes adalah 100%. Sebagai kesimpulan dari hasil analisis diatas bahwa kelengkapan dan keakuratan pengisian berkas rekam medis di RSU Universitas Muhammadiyah Malang masih kurang maka perlu di lakukan perbaikan agar tercapainya mutu pelayanan rekam medis. Untuk memudahkan petugas dalam perakitan dan pencarian formulir sebagai upaya meminimalisir tingkat ketidak lengkapan dan ketidak akuratan pengisian berkas rekam medis. di perlukan pengadaan pembatas dan sosialisasi kepada petugas medis maupun non medis guna menumbuhkan kedisiplinan dan tanggung jawab petugas.
