The purpose of this Final Project is to build useful information system for the existing work processes at High School 3 Sidoarjo Science Laboratory to make it easier, more efficient, more accurate and faster. This information system handles 7 (seven) processes including Practicum Scheduling Process, Practicum Rescheduling Process, Equipments, Materials and Chemical Substances Borrowing Process, Return Process, Equipments, Materials and Chemical Substances Stock Registering Process, Practicum Simulation Process and Report Making Process. The development of Integrated Science Laboratory Information System for High School 3 Sidoarjo required several stages. The first stage was acquaring data by conducting interviews. The second stage was identifying issues by making Document Flow Diagram and Fishbone Diagram. The third stage was creating process design by making Level Diagram and Data Flow Diagram (DFD). After that, system design continued with designing the database consisted Conceptual Data Model (CDM) with 25 entities, Physical Data Model (PDM) with 25 tables and input-output design. The forth stage was describing the system by drawing a Flow Diagram and implement it into a web-based information system using web programming. The last stage was testing the system using Black Box Testing method. Based on testing results Integrated Science Laboratory Information System for High School 3 Sidoarjo is in accordance with functional need means the system can handle work processes and make it easier, more efficient, more accurate and faster by computerized data.} }
