		abstract={Hospital Management and Information System (HMIS) is a communication information system processing the entire service process in the hospital. In Regional Hospital unit (RSUD) of Jombang Regency, HMIS has been implemented in one of functional units of Outpatient Services to assist running the business processes. In the operational activities of providing IT services to patients, it is not uncommon for HMIS of outpatient services to experience disruption. Thus, it hinders the business processes. Thus, a risk assessment is carried out to ensure that risks can be handled properly and they do not interfere with existing business processes. This research aimed to identify the risks of current outpatient service HMIS of RSUD Jombang Regency, identify the results of the risk assessment contained in the outpatient service HMIS of RSUD Jombang Regency, and map the IT process risks on the outpatient service HMIS of RSUD Jombang Regency as a step to mitigate risks that can be used as recommendations guide. The framework and methods used in this study are based on APO12 process (Manage Risks) and DSS01 process (Manage Operations). APO12 process for risk management with the stages of possible risk analysis, type of risks analysis, risk categories analysis, risk factors analysis and risk mapping to determine mitigation measures. Meanwhile, DSS01 process for identifying IT processes related to IT operational service fulfilment. The results of the risk assessment show that there were 14 risks (32.5%) at the high level. It also shows that the majority of risk comes from the category of staff operations (human error and malicious intent) and IT expertise and skills. Therefore, it is necessary to develop and implement written operational procedures (DSS01 Manage Operations) in order to minimize the risks caused by staff operations (human error and malicious intent). As for the risks caused by IT expertise and skills, it is necessary to increase the abilities and skills of employees in carrying out their responsibilities (APO07 Manage Human Resources). The output of this research is a document containing a list of potential risks, risk events, results of risk scenario analysis, results of risk assessments, and a list of risk mitigation measures. So, it is hoped that it may be used as a reference document of Unit SIM to carry out risk management so that risks can be managed for optimizing outpatient services in RSUD Kabupaten Jombang.}
