Abstract :
This research aims to determine the causes of being unscheduled
removal that occurs most frequently on JT8D-Series Engines, identify
failure modes based on the most frequent causes of unscheduled removal
occurs on the JT8D-Series Engine using the FMEA (Failure Mode and
Effect Analysis), as well as analyzing the level of reliability and rate
failure (failure rate) using the Weibull Analysis method. This research
carried out based on Unscheduled Removal Engine JT8D-Series data during
operational engine within a period of 5 years.
Based on data grouping, the causes of Unscheduled occur

The most common thing that occurs on JT8D-Series engines is turbine case failure.
In the failure mode analysis process using the FMEA method, we obtained
The highest RPN (Risk Priority Number) value is 486 with potential failure
the mode is a crack in the turbine blades. So it needs to be done preventively

crack on the turbine blade of the JT8D-Series Engine at certain time intervals

Based on analysis of the level of reliability and failure rate

1.27311 (Flight Hours) which indicates wear out failure.
Then we also obtained a Mean Time To Failure (MTTF) value of 1472.97
Flight Cycles or 1643.31 Flight Hours for turbine failure cases. Mark
JT8D-Series Engine reliability during MTTF (Mean Time To
Failure) is 43.18% (for analysis on Flight Cycles and 40.52% (for
analysis on Flight Hours). There needs to be additional preventive measures for
