Abstract :
Today's industry players are required to be able to work more efficiently and produce quality products amidst competition to meet increasingly complex consumer needs. The fulfillment of needs carried out by the company is by reducing defects in the products produced so that consumers will be happy and satisfied with the quality of the goods received. The purpose of this study is to determine the sigma level at PT. KUSUMA MULIA PLASINDO INFITEX, the factors that cause product defects, and determine an action plan in an effort to improve product quality with a kaizen approach. The methodology used is DMAIC with the stages of define, measure, analyze, improve, and control. At the define stage, the production process is mapped using the SIPOC diagram, Flow Diagram and Operational Process Map. Then the classification of types of defects and the definition of critical to quality (CTQ) are carried out. At the Measure stage, the calculation of the sigma value or level is carried out, the sigma level is 4.23? from a scale of 1? to 6? with a DPMO value of 3,129.347 meters and a P-Chart control map. Furthermore, the analyze stage is carried out using a fishbone diagram, and the factors that cause defects are obtained, including from the operator (man), machine, material, method and environment. At the Improve stage, the development of an action plan is carried out using the Kaizen and New Seven Tools approaches which include the Five-M Checklist and 5W + 1H (What, Why, Where, When, Who, How) concepts, Five-step plan, Affinity diagram, Tree diagram, Relationship diagram. With the main results of the corrective action plan, especially for workers, machines, materials and the environment. The main result of the New Seven Tools analysis is that the high level of product defects is caused by the lack of strict supervision carried out by the company on workers and machines.
