Abstract :
The World Health Organization (WHO) reported that Maternal Mortality Ratio (MMR) reaches 13.8 per 100,000 live births. In Indonesia, the MMR was still high in 2015, recorded at 305 per 100,000 live births, and the Neonatal Mortality Rate (NMR) was 22 per 1,000 live births. In Palembang, the MMR in 2019 was 20, and the NMR was 0.6 per 1,000 live births. Continuity of Care (COC) in midwifery is a comprehensive and continuous care model for pregnant women, during childbirth, postpartum, and for newborns, with the aim of preventing complications and reducing maternal and infant mortality rates that are still high in Indonesia. The COC in midwifery for Mrs. "K" atthe independent practice of midwife 'E' in Palembang started from June 30, 2022, until January 25, 2023, covering the early pregnancy period, childbirth, newborn care, postpartum care, and neonatal care. Data collection was conducted using both primary and secondary sources. The study design utilized a SOAP (Subjective, Objective, Assessment, Plan) case study. The results obtained for Mrs. "K" indicated a full-term pregnancy, normal childbirth, normal newborn condition, satisfactory postpartum period, and the neonate was within the normal range. In conclusion, the pregnancy was full-term, the childbirth was normal, the newborn was healthy, the postpartum period was satisfactory, and the neonate's condition was normal and within the physiological range. The recommendation for the practice area is to maintain and improve the quality of midwifery cares. Keywords: Midwifery care, Continuity of care
