Abstract :
It was reported that Maternal Mortality Ratio (MMR) in 2017 was 462 per 100,000 live births in low-income countries and 11 per 100,000 live births in high-income countries. Meanwhile, the Infant Mortality Rate (IMR) was 29 per 1,000 live births (WHO, 2021). MMR in Indonesia in 2015 was 390 per 100,000 live births, and IMR in 2021 was 154 per 1,000 live births. The number of MMR cases in South Sumatra in 2020 was 128, and IMR was 536 per 1,000 live births. In the city of Palembang, MMR in 2019 was 20 per 100,000 live births, and IMR was 1.2 per 1,000 live births. One of the Sustainable Developmental Goal (SDG) programs aims to reduce MMR to less than 70 per 100,000 live births and IMR to 25 per 1,000 live births by 2023. Data from Midwife "S" did not report any cases of maternal or infant mortality. Efforts to reduce MMR and IMR include implementing Continuity of Care (COC). This final project report aims to provide midwifery care with COC for Mrs. "W," 22 years old, at the independent practice of Midwife "S," starting from pregnancy, childbirth, newborn care, postpartum care, and neonatal care. COC was provided for Mrs. "W" in Midwife "S" from July 5, 2022, to April 1, 2023, covering pregnancy period, childbirth, newborn care, postpartum care, and neonatal care. Data collection was conducted using primary and secondary sources. A case study method was employed, with subjective, objective, assessment, and plan notes. The results showed that Mrs. "W" had a full-term pregnancy, normal childbirth, normal newborn condition, satisfactory postpartum period, and a healthy neonate. In conclusion, the pregnancy, childbirth, postpartum period, and neonatal period were within the physiological range. Some recommendations for the practice include maintaining and improving the quality of continuous midwifery care. Keywords: Midwifery care, Continuity of care
