Abstract :
World Health Organization (WHO) reported that the number of Maternal Mortality Rate (MMR) reached 216/100,000 Live Births (LB) in 2015. In 2030, Sustainable Development Programs (SDGs) set a target to decrease the MMR less than 70/100,000 LB. In Indonesia, the number of MMR was 305/100.000 in 2015. Also, the number of Infant Mortality Rate (IMR) was 27,566 cases in 2021. In South Sumatera, the number of MMR was 128 cases meanwhile the number of IMR was 454 cases in 2020. According to Soraya Independent Midwifery Practice (PMB), there was no any maternal or infant mortality case. The implementation of midwifery care of continuity care conducted by the midwives is one of efforts to take part in decreasing the MMR. This final report aims to provide midwifery care of continuity of care to 27-year-old Mrs. ?P? in Independent Midwifery Practice ?S? from pregnancy, labor, newborn baby, postpartum, to neonate. This case study used Subjective, Objective, Assessment, and Plan (SOAP) note and conducted in Independent Midwifery Practice ?S?. The research object was Mrs. ?P? G2P1A0 aged 27 years old and the object was provided with continuity of care from pregnancy care in trimester I, trimester II, and trimester III, labor, newborn baby, postpartum to neonate since July 23, 2022. The technique of collecting the data employed primary and secondary data. In conclusion, the midwifery cares to Mrs. ?P? from labor, newborn baby, postpartum, to neonate were still in physiological scope and normal range. However, in the Antenatal Care (ANC) III, it was found that there was a footling breech position. Next, the cephalic presentation was identified that enabled vaginal delivery. The researcher expects that this research report was used as a source of learning or reference to complete final report. The researcher also hoped to be a quality health worker in performing midwifery service. Key words: Midwifery Care, Continuity of Care (COC)
