Abstract :
The World Health Organization (WHO) states that high Maternal Mortality Rates (MMR) in several global regions reflects access inequality to health services quality and the gap between rich and poor. In 2020, maternal mortality in Indonesia happened to 4.627 people. The data showed that it was caused by several factors such as 1.330 cases of bleeding, 1.110 cases of hypertension in pregnancy, and 230 cases of blood flow disorders. To improve this condition, it is necessary to carry out a midwifery management approach with midwifery continuity of care with comprehensive and sustainable method of care. This final report aims to provide midwifery continuity of care for Mrs. "P" starting from pregnancy, childbirth, newborn baby, neonates, and family planning. In this final report, the midwifery continuity of care for Mrs. "p" in Midwife Ella independent practice Palembang was conducted from June 9, 2022 to February 4, 2023. The data collection technique involved primary and secondary data by implementing the midwifery continuity of care approach as the method of the study. It was concluded that the condition of pregnancy, childbirth, newborn baby, neonates, and family planning of Mrs "P" was diagnosed to be within normal limits. Moreover, the plan and assessment for midwifery continuity of care of Mrs "P" was done appropriately. It is suggested that the Independent Practice may maintain the midwifery care system based on the service standard.
