Abstract :
This study was conducted to determine the process by which there is defective product with Six Sigma method are then carried out by means of continuous improvement Kaizen implementation in the form of Five-Step Plan, and the Five-M Checklis. After processing the data obtained DPMO value of 7291 in January 2014 from a total defect 95700. The company is located at 3.1 sigma level with CTQ (Critical To Quality) most defects that cause Over Size. From the analysis it can be concluded that the main cause of disability is a human factor, machinery and tools based on the implementation of kaizen, the main policies that should be run by the companies that control or tighter controls.
