Abstract :
Medication error is an adverse event for patients due to drug use during the
handling of health workers who can actually be prevented (Kepmenkes RI, 2004).
Since November 1999 the incidence of medication error in hospitalized patients in
the United States has reached 7000 cases according to the Institute of Medication
(IOM) report. While in Indonesia the incidence of medication errors has not been
recorded clearly and accurately. The purpose of this study was to determine the
potential incidence of medication errors in the prescribing phase in the treatment
services at Bangsal Melati 2 RSUD Dr. Moewardi. This research is a prospective
descriptive study. This study used a random sampling method with 250 prescription
samples taken in March-April 2020 at Apotek rawat Inap Reguler RSUD Dr.
Moewardi. The potential number of medication error events in the prescribing
phase is 48.80%. Where errors include: unclear prescription writing by 3.20%, the
inaccuracy of the doctor's identity 40.00%, the imprecision of the patient's identity
33.20%, no dosage form of drug 14.40%, no drug potential 2.40%, no drug
frequency 2.40%, no rule of taking medicine 2.40%, drug duplication 1.60%,
potential of drug allergy 0.00%, drug interaction 8.40%, and no patient weight
47.20%. There were still errors in prescribing that had the potential for medication
errors in Bangsal Melati 2 RSUD Dr. Moewardi. The results of this study are
expected to help improve the quality of service to patients and prevent medication
errors in the prescribing phase.
