Abstract :
Prolanis is a health service program that is implemented in an integrated
manner for health care for sufferers of chronic diseases such as diabetes mellitus.
The purpose of this study was to determine the profile profile of diabetes mellitus
patients, medical consultation and education activities for Prolanis participants at
the Kimia Farma Karanganyar Pharmacy. The population as well as the sample of
the study are patients with diabetes mellitus Prolanis participants in the Pharmacy
Chema Pharmacy Karanganyar as many as 70 people. Data collection using a
questionnaire. Data analysis techniques with univariate analysis. The results of the
study are the profiles of participants with diabetes mellitus in terms of age
dominated by age 51-60 years (51.4%), of the sex dominated by women (62.9%), in
terms of education dominated by high school education level equivalent (51.4%), in
terms of work many do not work (40.0%). The medical consultation for Prolanis
participants at the Kimia Farma Karanganyar Pharmacy was already good with the
highest percentage of blood pressure checks and evaluation of disease progression
through recording in the patient's health status monitoring book (74.3%) while the
lowest percentage of the time contract was with the health worker before
consultation (34.3%). The implementation of education for Prolanis participants at
the Kimia Farma Karanganyar Pharmacy was good with the highest percentage of
education about complications and self-care for DM type 2 (47.1%) and the lowest
percentage about the symptoms of DM type 2 (37.1%)
