KaryaTulis Ilmiah Sekolah Tinggi Ilmu Administrasi (STIA) Malang Program Studi Diploma III Perekam Medis dan Informasi Kesehatan. Dosen Pembimbing Eka Agustiningtias.,S.AB.,M.,AB Puskesmas sebagai sarana pelayanan kesehatan yang menyediakan fasilitas pelayanan kesehatan pada tingkat pertama, di harapkan dapat memberikan kegiatan-kegiatan pelayanan untuk masyarakat sesuai dengan tugas dan wewenangnya. Penelitian dilakukan di Puskesmas Dau kabupaten Malang mengenai Analisis Faktor-Faktor Penyebab Ketidaklengkapan Pengisian Berkas Rekam Medis Pasien Rawat Jalan Di Puskesmas Dau. Tujuan Penelitian : Untuk mengetahui bagaimana pengisian berkas rekam medis pasien rawat jalan di Puskesmas Dau.Untuk Mengetahui faktor penyebab ketidaklengkapan pengisian berkas rekam medis pasien rawat jalan di Puskesmas Dau Kabupaten Malang. Jenis Penelitian Yang digunakan dekskriptif dengan pendekatan kualitatif dan fokus penelitian adalah menganalisis faktor-faktor penyebab ketidaklengkapan pengisian berkas rekam medis pasien rawat jalan di Puskesmas Dau. Teknik pengumpulan data dengan metode observasi, wawancara dan dokumentasi. Hasil penelitian yaitu: Faktor-Faktor Penyebab Ketidaklengkapan Pengisian Berkas Rekam Medis Pasien Rawat Jalan Di Puskesmas Dau, dengan persentase kelengkapan 96,62% dan ketidaklengkapan 3,38%. Ditemukan kesibukan dokter dalam satu hari tidak hanya memeriksa satu pasien saja, melainkan beberapa pasien sehingga dokter tidak bisa fokus dalam mengisi berkas rekam medis pasien rawat jalan, petugas rekam medis khususnya di bagian assembling terkadang lupa dalam melengkapi dan mengecek pengisian berkas rekam medis yang tidak lengkap. Dilihat dari kelengkapan pengisan berkas rekam medis pada Puskesmas Dau Kabupaten Malang, dapat disimpulkan bahwa kelengkapan pengisian berkas rekam medis cukup baik dikarenkan persentase kelengkapan lebih tinggi dari pada persentase ketidaklengkapan. Saransebaiknya bagi petugas rekam medis khususnya di bagian Assembling perlu adanya peningkatan kesadaran dalam melengkapi berkas rekam medis. The study was conducted at Dau Public Health Center in Malang Regency on Analysis of Factors Causing the Incompleteness of Medical Records File Outpatients in Dau Health Center. Research Objectives: To find out how to fill out the medical record file for outpatients in Dau Health Center. To find out the factors causing incompleteness in filling out the medical record file for outpatients in Dau Health Center in Malang Regency. Type of Research The descriptive study used a qualitative approach and the focus of the study was to analyze the factors that caused the incompleteness of filling out the medical record file for outpatients at Dau Health Center. Data collection techniques with the method of observation, interviews and documentation. The results of the study are: the factors incompleteness in filling out the medical record file for outpatients in Dau Public Health Center , with the percentage of completeness 96,62% and incompleteness of 3,38%. Doctors were found busy in one patient, but several patients so that doctors could not focus on filling out the medical record file for outpatients, medical records officers especially in the assembling section sometimes forgot to complete and check the filling of incomplete medical record files. Judging from the completeness of filing medical record files at the Dau Health Center in Malang Regency, it can be concluded that the completeness of filling medical record files is quite good because the percentage of completeness is higher than the percentage of incompleteness. Suggestions should be for medical record officers, especially in the Assembling section there needs to be increased awareness in completing the medical record file.
