Abstract :
ABSTRAK Penyusutan dan pemusnahan berkas rekam medis dilakukan untuk mengurangi berkas rekam medis yang semakin bertambah sebagai hasil dari kegiatan administrasi puskesmas. Pemusnahan berkas rekam medis dilakukan secara bertahap yaitu dimulai dari pemilahan, pemindahan, penilaian, dan pemusnahan. Puskesmas Ciptomulyo belum mempunyai jadwal tetap dan SOP tentang pemusnahan atau buku pedoman pemusnahan, sehingga peneliti tertarik untuk mengambi penelitian ini. Tujuan dari penelitian ini adalah untuk mengetahui identifikasi berkas rekam medis aktif dan inaktif, pemusnahan berkas rekam medis inaktif, fasilitas yang digunakan dalam pelaksanaan pemusnahan, serta pelayanan saat ini di Puskesmas Ciptomulyo kota Malang. Jenis penelitian ini adalah explanatory research. Data diperoleh melalui hasil wawancara dengan informan seorang petugas rekam medis Puskesmas Ciptomulyo Kota Malang yang dilakukan pada tanggal 08-15 Agustus 2019 serta observasi pada Unit Rekam Medis di Puskesmas Ciptomulyo Kota Malang. Hasil penelitian ini menjelaskan bahwa identifikasi berkas rekam medis aktif dan inaktif di Puskesmas Ciptomulyo belum sesuai SOP, prosedur pemusnahan berkas rekam medis inaktif belum mempunyai SOP dan jadwal pemusnahan, fasilitas yang digunakan dalam pelaksanaan pemusnahan berkas rekam medis inaktif hanya mesin pencacah dan tidak ada ruang khusus untuk menyimpan berkas rekam medis inaktif, serta pelayanan saat ini memiliki beberapa permasalahan seperti kurangnya petugas rekam medis untuk kegiatan retensi dan tidak adanya ruang khusus untuk menyimpan berkas-berkas rekam medis inaktif yang telah diretensi sehingga berkas rekam medis inaktif menumpuk dan mengurangi kelancaran pelayanan pasien. Kata kunci : identifikasi berkas rekam medis, prosedur pemusnahan berkas rekam medis, fasilitas, pelayanan. Ciptomulyo Public Health Center does not have a fixed schedule and SOP on extermination or an extermination manual, thus the researcher are interested in taking this research This study aimed to determine the identification of active and inactive medical record files, the destruction of the inactive medical record files, the facilities used in implementing the destruction, and the current service at the Ciptomulyo Public Health Center in Malang. The results of this study explained that the identification of active and inactive medical record files at the Ciptomulyo Health Center was not in accordance with the SOP, the procedure for destroying the inactive medical record files did not have an SOP and the schedule for destruction, the facilities used in implementing the destruction of inactive medical record files were only the enumerator machine and there was no special room to store inactive medical record files, and the service currently has several problems such as lack of medical records officers for retention activities and lack of special space to store inactive medical record files that have been retained so that inactive medical record files accumulate and reduce the smooth operation of patient services.
