Abstract :
ABSTRAK Kelengkapan pengisian berkas rekam medis menjadi hal yang sangat penting karena jika ada berkas rekam medis yang tidak terisi dengan lengkap akan mengakibatkan berkurangnya informasi terkait pasien, terutama pasien rawat jalan peserta BPJS apabila berkas rekam medis belum terisi lengkap akan mengakibatkan masalah yang berkaitan dengan mutu pelayanan serta akan berbanding terbalik dengan SOP yang telah ditetapkan. Hal ini akan mempengaruhi kurang optimalnya pelayanan yang diberikan terhadap proses pengobatan dan penyembuhan kepada pasien. Setiap kegiatan pelayanan medis harus mempunyai rekam medis yang lengkap dan akurat untuk setiap pasien, setiap dokter dan dokter gigi wajib mengisi rekam medis dengan benar, lengkap dan tepat waktu dalam bidang kesehatan. Untuk mengetahui ketidaklengkapan dalam pengisian berkas rekam medis pasien rawat jalan peserta BPJS di Puskesmas Mojolangu Malang maka tujuan penelitian ini yaitu ingin mengetahui tentang ketidaklengkapan pengisian dan akibat dari pengisian berkas rekam medis pasien rawat jalan peserta BPJS. Metode yang digunakan dalam penelitian ini adalah jenis penelitian kuantitatif dengan pendekatan deskriptif, untuk pengumpulan data metode yang digunakan yaitu metode pengumpulan data secara observasi dan dokumentasi dan metode analisa yang digunakan adalah analisa deskriptif. Hasil yang didapatkan dari penelitian ketidaklengkapan pengisian berkas rekam medis pasien rawat jalan peserta BPJS di Puskesmas Mojolangu Malang sudah terlaksana dengan baik terbukti pada item nomor rekam medis, nama pasien, alamat pasien,tanggal lahir pasien dan tanggal pemeriksaan sudah mencapai 100% akan tetapi masih ada yang belum terisi dengan lengkap yaitu pada item nomor handphone/hp angka ketidaklengkapan mencapai 47% dan hasil anamnesis angka ketidaklengkapan mencapai 25%, oleh sebab itu petugas di tempat pendaftaran dan petugas di tiap poli (dokter,perawat maupun bidan) harus lebih teliti dan tidak lupa untuk melengkapi berkas rekam medis pasien serta adanya sosialisasi kepada petugas tentang pentingnya kelengkapan dalam pengisian berkas rekam medis pasien rawat jalan. This will affect the suboptimal service provided to the process of treatment and healing to patients. Every medical service activity must have a complete and accurate medical record for every patient, every doctor and dentist must fill in the medical record correctly, completely and on time in the health field. To find out the incompleteness in filling out the BPJS outpatient medical record file at Mojolangu Health Center Malang, the purpose of this study is to find out about the incompleteness of filling and the consequences of filling out the BPJS outpatient medical record file. The method used in this research is a type of quantitative research with a descriptive approach, for data collection the method used is the method of collecting data by observation and documentation and the analysis method used is descriptive analysis. The results obtained from the incomplete research of filling out the medical record file for outpatients of BPJS participants in Mojolangu Public Health Center Malang have been well implemented as evidenced in the items of medical record number, patient's name, patient's address, patient's date of birth and date of examination has reached 100% but it still exists that has not been filled in completely, namely the item number of mobile / cell phone the incompleteness number reaches 47% and the results of the history of incompleteness figure reach 25%, therefore the officer at the place of registration and the officer in each poly (doctor, nurse or midwife) must be more thorough and not forgot to complete the patient's medical record file as well as the socialization to officers about the importance of completeness in filling out the patient's medical record file.
