Abstract :
ABSTRAK Puskesmas adalah Pusat pelayanan kesehatan yang bertanggungjawab menyelenggarakan pembangunan kesehatan diwilayah kecamatan.dalam menyelenggarakan pembangunan kesehatan masyarakat puskesmas dikategorikan menjadi dua jenis yaitu Puskesmas Non rawat Inap dan rawat inap.medis juga berguna sebagai bukti tertulis atas tindakan-tindakanpelayanan terhadap seseorang pasien, juga mampu melindungikepentingan hukum bagi pasien yang bersangkutan, Puskesmas maupun dokterdan tenaga kesehatan lainnya, apabila dikemudian hari terjadi suatu hal yang cepat dan tepat waktu sesuai dengan ketentuan yang telahditetapkan oleh puskesmasPenyediaan berkas rekam medisyang cepat pun merupakan salah satu faktor yang dapat mempengaruhikepuasan pasien.Semakin cepat pula pasien mendapatkan pelayanan kesehatan di Puskesmas.merupakan spesifikasi teknis tentang tolak ukur pelayanan yang minimum yang diberikan oleh badan layanan umum kepada masyarakat. Berdasarkan keputusan menteri kesehatan RI No.129/Menkes/SK/II/2008 tentang Standar pelayanan minimal rekam medis terdiri dari kelengkapan pengisian rekam medis 24 jam setelah selesai pelayanan 100%, kelengkapan informed consent setelah mendapatkan informasi yang jelas 100%, waktu penyediaan dokumen rekam medis pelayanan rawat jalan ?10 menit, waktu penyediaan dokumen rekam medis pelayanan rawat inap ?15 menit. Pelayanan kesehatan dkatakan baik apabila sistem pengolahan rekam medis dalam mendapatkan kembali rekam medis cepat dan tepat waktu. Pada proses penyediaan dokumen rekam medis puskesmas dau menetapkan waktu penyedian dokumen rekam medis dari pasien didaftar sampai dokumen rekam medis didistribusi maka waktunya yaitu 10 menit sesuai dengan standar pelayanan minimal di puskesmas. Dari hasil penelitian yang saya amati adalah 1 orang petugas rekam medis dengan dengan 25 berrkas rekam medis dalam waktu 10 menit ,jadi berkas dokumen rekam medis pasien baru baik tepat waktu maupun tidak tepat waktu sebanyak9 dokumen .pasien lama tepat waktu dan tidak tepat waktu yaitu 12 dokumen dan pasien SKD sebanyak 4 dokumen.jadi jumlah seluruh dokumen rekam medis yaitu sebanyak 25 dokumen dengan interval waktunya sebagai berikut. Kata kunci : Standar Pelayanan Minimal penyediaan dokumen rawat jalan. ABSTRAC Puskesmas is a unit tecnichal implementation district or city helth office which is responsible for organizing health development in the district area in carrying out community health development puskesmas are categorized into two types namely non inpatient and inpatient puskesmas.medical records are alsouseful as written evidence of acts of service to a patient,also able to protect the legal interest of patients concerned,health centers and doctor and other health workers,in the future something happens that is fast and on time in accordance with the conditions set by the puskesmas, the provision of medical records files that is fast is also one of the factors that influence patient satisfction. The faster the patient gets health services at the puskesmas.is a technical specification of the minimum service benchmarks provided by public service bodies to the community. Based on the decision of the minister of health of the republic of indonesia no.129/Menkes/SK/II/2008 Regarding the minimum service standard for medical records consisting of completeness of filling medical records 24 hours after completion of service 100%,completeness of informed consent after completing obtaining clear information 100%,time for providing medical records for out patient services <10 minute,tie for providing medical records for inpatient services < 15 minute.health service are said to be good if the medical record processing system in getting medical records back quickly and on time. In the process of providing medical records documents at the puskesmas Dau and setting the time for the provision of medical record documents from patients on the list until the medical record documents are distributed then the time is 10 minutes according to the minimum serivce standarts at the puskesmas. From the results of the study that i observed was 1 person medical records officer with 25 medical record files within 10 minutes.so the new patient?s medical record file either on time or not time 9 dokuments,old patientsare on time ,12 documents,and patients doctor?s certificate 4 documents.so the total number of medical record documents is as many as 25 documents with the following time intervals.
