Abstract :
ABSTRACT A complete medical record is if the patient identification data is filled in, important reporting, authentication and using good documentation procedures. Medical records that must be available in a health service facility must contain all information relating to the services that have been provided to patients. Completeness of the medical record is important for patients, medical, legal proceedings, and health insurance. Has become the responsibility of the doctor in carrying out medical practice must make a medical record. The purpose of this study was to determine the process of completing forms and provide an overview of the importance of form design in providing information for each item on the inpatient medical record form at Kanjuruhan Regional Hospital. This type of research is descriptive in nature, data taken from the medical record unit of Kanjuruhan Regional Hospital Malang, in the Medical Records Document Storage (Filling) section. The data collected is derived from inpatient medical record documents, then identification of the percentage of completeness and incompleteness of the inpatient medical record form is identified. Data is presented in tabular form to assess the level of completeness of inpatient medical record forms at the Kanjuruhan District Hospital in Malang. From the results of the study, the Percentage of Completeness Filled in Medical Records (KLPCM) in Kanjurhan District Hospital Malang in March - May 2019, starting from the patient's name, the patient's diagnosis up to the doctor's signature and the doctor's name tends to be incomplete. This can be seen in the percentage of completeness of each item. Medical records are incomplete in each room and each item reach 84.57% in March, 83.59% in April, 45.13% in May 2019 and only a few percent form are complete. It clearly shows that the level of completeness has not yet reached the 100% complete target. Keywords: Incompletness, Form, Medical Record,Inpatient, SNARS Accreditation. ABSTRAK Rekam medis yang lengkap adalah jika terisinya data identifikasi pasien, pelaporan penting, autentikasi dan menggunakan tata cara pendokumentasian yang baik. Rekam medis yang harus tersedia disuatu sarana pelayanan kesehatan, harus memuat seluruh informasi yang berkaitan dengan pelayanan yang telah diberikan kepada pasien. Kelengkapan rekam medis ini penting bagi pasien, medis, proses hukum, dan asuransi kesehatan. Sudah menjadi tanggung jawab dokter dalam menjalankan praktik kedokteran wajib membuat rekam medis. Tujuan penelitian ini adalah untuk mengetahui proses keterisian formulir dan memberikan gambaran tentang pentingnya desain formulir dalam memberikan informasi untuk setiap itemnya pada formulir rekam medis rawat inap di RSUD Kanjuruhan. Jenis penelitian ini bersifat deskriptif, data yang diambil dari unit rekam medis RSUD Kanjuruhan Kabupaten Malang dibagian Penyimpanan Dokumen Rekam Medis (Filling). data yang dikumpulkan berasal dari dokumen rekam medis rawat inap, kemudian dilakukan identifikasi perhitungan prosentase kelengkapan dan ketidaklengkapan lembar formulir rekam medis rawat inap. Data disajikan dalam bentuk tabel untuk menilai tingkat kelengkapan keterisian formulir rekam medis rawat inap di RSUD Kanjuruhan Kabupaten Malang. Dari hasil penelitian, Prosentase Kelengkapan Angka Ketidak Lengkapan Pengisian Catatan Medis (KLPCM) di RSUD Kanjurhan Kabupaten Malang pada bulan Maret ? Mei 2019, mulai dari Nama Pasien, diagnosa pasien sampai dengandia tanda tangan dokter dan nama dokter cenderung belum lengkap. Hal ini dapat dilihat pada Prosentase kelengkapan setiap itemnya. Rekam Medis yang kurang lengkap pada setiap ruangan dan setiap item mencapai 84,57% pada bulan Maret, 83,59% pada bulan April, 45,13% pada bulan Mei 2019 dan hanya beberapa persen DRM yang lengkap. Jelas menunjukkan kalau tingkat prosentase kelengkapannya belum mencapai target 100% lengkap. Kata Kunci : Ketidaklengkapan, Formulir, Rekam Medis, Rawat Inap, Akreditasi SNARS
