REVIEW OF THE IMPLEMENTATION OF MEDICAL RECORDING FILES AT BAPTIS KEDIRI HOSPITAL

Abstract :
Hospital is an instaion of health services for the health of the students to complete plannings that provide road, hospital and emergency settings. The hospital can function optimally if it is supported by professional staff / human resources. Medical Record is a file that contains records of service from beginning to end of treatment. All medical record files must be stored in a medical record document storage rack (filling). Medical record file retention is the activity of shrinking or reducing the medical record file from the storage rack by moving the inactive medical record file, which is currently on the active shelf to the inactive rack. To support the efficiency of storage space and reduce the burden of storing medical record files and capture forms that have a use value, it is necessary to carry out the process of retention of medical record files. The purpose of this research is to find out and explain the implementation of medical record file retention in Kediri Baptist hospital, with the object of research being carried out at Kediri baptism hospital. This type of research is descriptive using a qualitative approach in the form of interviews, observations and use of documents. Based on the observation results of medical record file retention in Kediri Baptist Hospital, it shows that the results of the implementation of retention are in accordance with standard operating procedures, according to director's regulation No.38 / Per.Dir / RSBK / VIII.2017. Although in the implementation that is in accordance with the SOP, but there are some problems that were found including the lack of specialized human resources for the maintenance / management of inactive medical record files, there is no dividing shelf between active medical record files and inactive medical record files, the time needed for the retention process less effective. Based on some of these problems the authors provide 3 recommendations for retention of medical record files at the Baptist hospital in Kediri including by adding officers who can manage inactive medical record files and conducting socialization so that officers better understand how to manage medical records so there is no error in the retention process. In addition, filling officers should sort out the active and inactive medical record file so that there is no buildup of medical record file on the active medical record file rack, as well as increase work time more effectively and efficiently so that the retention process can run smoothly. Rumah sakit dapat berfungsi secara maksimal jika di dukung dengan tenaga/SDM (sumber daya manusia) yang profesional. Rekam Medis adalah berkas yang berisikan catatan pelayanan pada pasien dari awal berobat hingga akhir. Semua berkas rekam medis harus disimpan ke dalam rak penyimpanan dokumen rekam medis (filling). Retensi berkas rekam medis adalah kegiatan penyusutan atau pengurangan arsip rekam medis dari rak penyimpanan dengan cara memindahkan arsip rekam medis inaktif, yang saat ini berada di rak aktif menuju ke rak inaktif. Untuk menunjang efisiensi ruang penyimpanan serta mengurangi beban penyimpanan berkas rekam medis dan mengabadikan formulir-formulir yang memiliki nilai guna maka perlu dilakukan proses retensi berkas rekam medis.Tujuan dalam penellitian ini adalah untuk mengetahui dan menjelaskan pelaksanaan retensi berkas rekam medis di rumah sakit Baptis Kediri, dengan objek penelitian yang dilaksanakan dirumah sakit baptis Kediri. Jenis penelitian yang dilakukan adalah deskriptif dengan menggunakan pendekatan kualitatif yang berupa wawancara, pengamatan dan pemanfaatan dokumen. Berdasarkan hasil observasi retensi berkas rekam medis di rumah sakit baptis kediri, menunjukan bahwa hasil pelaksanaan retensi sudah sesuai dengan standar operasional prosedur, sesuai peraturan direktur No.38/Per.Dir/RSBK/VIII.2017. Walaupun dalam pelaksanaan yang sudah sesuai dengan SOP namun ada beberapa permasalahan yang di temukan diantaranya kurangnya tenaga SDM khusus untuk pemeliharaan/pengelolaan berkas rekam medis inaktif, tidak adanya rak pemisah antara berkas rekam medis aktif dan berkas rekam medis inaktif, waktu yang dibutukan untuk proses retensi kurang efektif. Berdasarkan beberapa permasalahan tersebut penulis memberikan 3 rekomendasi retensi berkas rekam medis di rumah sakit baptis kediri diantaranya dengan menambah petugas yang dapat mengelola berkas rekam medis inaktif dan melakukan sosialisasi agar petugas lebih memahami cara pengelolaan rekam medis sehingga tidak terjadi kesalahan dalam proses retensi.
