ANALISIS KELENGKAPAN IDENTITAS PASIEN UNIT RAWAT INAP PADA PENDAFTARAN PASIEN BARU DI RUMAH SAKIT PANTI WALUYA SAWAHAN MALANG(JANUARI 2019 – JUNI 2019)

Abstract :
ABSTRAK Kelengkapan pengisian formulir terdapat formulir yang kurang lengkap seperti identitas pasien, diagnosa, pemeriksaan fisik, pemeriksaan penunjang, tanda tangan dokter dan nama dokter dalam pengisiannya sehingga sangat berpengaruh terhadap informasi yang dibutuhkan pasien dan dalam laporan yang dibuat oleh rumah sakit tersebut. Jenis penelitian yang digunakan yaitu deskriptif, metode pengumpulan data meggunakan wawancara dan checklist.Penelitian ini mengambil populasi dari jumlah keseluruhan pasien berdasarkan data global yang diperoleh dari pihak Rekam Medis yang kemudian dihitung masing-masing perbulannya.Sedangkan sampelnya digunakan sebagai bahan menghitung angka kelengkapan dan ketidaklengkapan isi identitas pasien berupa data pribadi dan data sosial. Dari hasil penelitian didapatkan kelengkapan pengisian identitas pasien pada pendaftaran pasien baru rawat inap periode Januari?Juni 2019 belum mencapai target 100% yakni dibulan Januari 99,8%, Maret 99,5%, dan juga Mei 99,6%. Berdasarkan hasil penelitian dapat disimpulkan bahwa dalam pengisian identitas pasien rawat inap baik identitas pribadi maupun identitas sosial pada periode bulan januari-juni 2019 masih ada yang tidak terisi dengan lengkap sesuai dengan standar yang ditentukan yakni 100%. Hal ini disebabkan oleh beberapa faktor yaitu pasien lupa membawa kartu identitas (KK,KTP,SIM,Paspor dan juga kartu pengenal lainya. While the sample is used as material counting the number of completeness and incompleteness of the patient's identity content of personal data and social data. From the results of the research obtained the completeness of patient identity in the enrollment of new patient hospitalization period January ? June 2019 has not reached the target of 100%, namely in January, March, and also May. Based on the results of the study can be concluded that in the identification of the identity of the patient in both personal identity and social identity in the period of January-June 2019 there is still a complete unfilled in accordance with the standards Determined to be 100%. This is due to several factors, the patient forgot to bring identity card (KK, ID card, driver's license, passport, and also other ID card.
