Abstract :
Background: Head and neck tumors can involve both soft tissue and hard tissue with a global incidence of 600,000 cases per year and men are more at risk than women. Low public awareness and limited access to health facilities can be a factor for patients arriving at Type A Hospitals with advanced or large-sized tumors that require wide excision and cause large defects that interfere with function and aesthetics. Free flaps are the gold standard for reconstruction, but research related to this procedure is still difficult to find in Indonesia.
Methods: Patients of the Reconstructive and Aesthetic Plastic Surgery Division who underwent free flap reconstruction after head and neck tumor ablation at Dr. Mohammad Hoesin Palembang for the period 2017 ? 2022.
Results: From this study, there were 27 post-ablation head and neck tumor patients who underwent free flap reconstruction with the highest distribution in the age group <50 years (70.4%; n=19), male sex (55.6%; n= 15), and primary education (55.6%; n=15), diagnosed ameloblastoma (37%; n=10), mandibular region (40.7%; n=11), reconstructed with FFF (37%; n= 10) and RFFF (37%; n = 10), the average size of FFF 31 cm2, RFFF 68.72 cm2, and ALT 266.71 cm2, facial arteries and veins (85.2; n = 23) , end-to-end anastomotic technique (100%; n=27), ischemic time <60 minutes (81.5%; n=22), duration of surgery 11 ? 15 hours (77.8%; n=21 ), and the free flap viability rate was 63% (n=17).
Conclusion: Free flap reconstruction is the best option for patients after ablation of head and neck tumors because it can cover large defects involving more than one tissue component which is performed safely, effectively, and provides good functional and esthetic results in patients of all ages thereby providing opportunities for Oncology Surgeons to remove tumors optimally without worrying about large defects, functions, and aesthetics that will interfere with the patient's quality of life
